Constellation Regional Learning Collaborative: Impact Report

The Constellation Regional Learning Collaborative (CRC) Impact Report highlights how community-clinical partnerships in southeastern North Carolina are advancing equitable cardiovascular care. Through a CDC-funded initiative, Novant Health New Hanover Regional Medical Center strengthened blood pressure management, expanded stroke education, and integrated community paramedicine and care coordination to improve outcomes across rural communities.

With over 1,200 stroke education touchpoints delivered and new pathways created for post-discharge support, this initiative is building sustainable systems change where it is needed most.
