Constellation Quality Health is launching a Project ECHO Hypertension and Cardiovascular Disease Interdisciplinary ECHO series as our inaugural ECHO series.
This series seeks to engage primary care providers, clinicians, team-based care members, and community partners to address hypertension and cardiovascular disease health outcomes while reducing disparities and cost. The series will also integrate equitable and culturally appropriate care delivery, engaging in the whole-person approach to improve health outcomes.
Our Hypertension and Cardiovascular Disease Interdisciplinary ECHO series will cover topics such as:
- Hypertension evidence-based treatments and best practices
- Social determinant barriers in treating hypertension
- Health equity and culturally appropriate care delivery
- Utilizing data and social determinant CPT Z codes to impact
health outcomes - Working with community partners to address whole-patient care
