About the CRC

Launched by Constellation Quality Health in 2023, the Constellation Regional Collaborative (CRC) is a five-year cooperative agreement funded by the Centers for Disease Control and Prevention (CDC), designed to reduce cardiovascular disparities and improve health equity across the Southeast. 

Our Aims

  • Address cardiovascular health disparities in communities facing the greatest burden of disease.
  • Direct efforts across North Carolina, South Carolina, Georgia, and Florida counties with hypertension rates of 53% or higher.
  • Strengthen team-based care through coordinated clinical and community support.
  • Connect patients and community members to the support they need by linking community resources with clinical care.
  • Leverage referral networks and Community Health Workers to close referral loops and ensure needs are met.
  • Support providers in improving hypertension and cardiovascular care measures. 

OUR FOUR PROGRAM INITIATIVES

The Constellation Community Care Hub is a Community Health Worker-led centralized network that brings together healthcare providers and community-based organizations to address barriers to care for at-risk cardiovascular patients and community members.

The Hypertension & Cardiovascular Disease Interdisciplinary ECHO Series offers free, monthly All Teach, All Learn virtual sessions that bring healthcare providers, community organizations, and other partners together to share knowledge and practical strategies.

The In Clinic Pharmacist Pilot is elevating the role of pharmacists as integral members of the healthcare team, this pilot supports them in practicing at the top of their license, providing targeted medication and care recommendations that lead to improved health outcomes.

The Constellation Regional Learning Collaborative is an annual initiative brings together healthcare providers and community-based organizations across the Southeast to strengthen their capacity to prevent, detect, and manage hypertension and cardiovascular disease.
