Constellation Regional Learning Collaborative

The Constellation Regional Learning Collaborative (CRLC) is a 12-month, cohort-based initiative offered annually in North Carolina, South Carolina, Georgia and Florida. The CRLC brings together healthcare providers, community-based organizations, and public health leaders to drive measurable improvements in hypertension and cardiovascular disease (CVD) outcomes.

Goals of the CRLC

Improve Hypertension and CVD Management
Equip teams with practical, evidence-based interventions for controlling blood pressure and improving heart health.

Advance Health Equity
Help participants identify and respond to social, structural, and systemic factors that influence health, ensuring interventions meet the unique needs of the communities they serve.

Build Capacity Across the Care Team
Support workforce development through self-paced learning, coaching, and skill-building in quality improvement and clinical best practices.

Support Sustainable Community-Based Solutions
Encourage scalable programs rooted in local context and driven by patient-centered design.

Strengthen Regional Collaboration
Leverage tools like Vega, a bi-directional referral platform, to connect clinical care with social supports, improving outcomes through shared accountability.

The Southeastern United States has the unfortunate reputation of having the highest stroke and heart disease mortality rates in the nation. We anticipate a robust response from our Carolina providers and community in joining the Constellation Regional Collaborative to address inequalities in health care and reverse this trend.
