Raleigh, NC – The CCME Foundation, a nonprofit organization supporting health care access and improvement in the Carolinas, is accepting grant applications from eligible nonprofit organizations that work to improve health care for individuals and communities through measurable outcome improvement projects.
The yearly grants range from $5,000 to $50,000 and fund projects that impact health care improvements and access for individuals and communities, particularly the uninsured and underserved. Eligible nonprofits are invited to apply for funding April 1 through June 15, 2025. Grant recipients will be announced in early August 2025.This year, the Foundation is implementing a “Promising Practices Framework” to encourage partnership and innovation at the grassroots level, primarily in rural and sparsely suburban populations and/or in under and underserved populations within the following priority focus areas: post-acute care, behavioral health, chronic disease management, and workforce development. Urban-based organizations are also encouraged to apply. More information on the grants and framework can be found on the CCME Foundation webpage.
“We are excited for 2025, as it marks a five-year milestone for the CCME Foundation in supporting people and communities in the Carolinas in health care improvement initiatives,” said Steven Martin, president and CEO of Constellation Quality Health, the nonprofit organization that supports the CCME Foundation. “Through the Foundation’s grants, populations, especially those underserved and most in need, demonstrably experience better health care and improved lives.”
The Foundation considers applicants with current 501(c)(3) status, including faith-based, hospital/provider/clinic, educational institutions, and community service organizations. It will not fund reimbursable direct patient care services, facility construction or remodeling of facilities, lobbying, and fundraising activities. Awardees are required to submit two online progress reports throughout the contract period. The CCME Foundation Board of Directors Review Committee oversees the grant process and determines recipients.
