What you’ll do:

The Community Health Worker (CHW) focuses on community-clinical linkage by bridging health care and community services, helping patients connect with essential resources and health education in a collaborative care model. The ideal candidate will assist in program development and possess strong leadership skills with a commitment to fostering community engagement and empowerment.

 

 

We expect you to:

  • Oversee the integration with health care providers and community-based organizations (CBOs) to navigate patient referrals and close bidirectional referral loops while maintaining compliance with state and federal regulations.
  • Conduct screenings and assessments to identify patient and household health needs, providing culturally tailored education on cardiovascular and related health topics.
  • Assist in the development and implementation of an integrated community care model that connects health services, social support, and community resources.
  • Build and sustain partnerships with community stakeholders to enhance health literacy and access to care, while also conducting outreach activities to educate the community on available health resources/services.
  • Act as a subject matter expert on CHW program workflow and care model, providing leadership, guidance, mentorship, and supervision of CHWs.

 

Regional travel is REQUIRED in South Carolina to meet patients, engage with community stakeholders, attend conferences, and relationship building; therefore, candidates must reside in SC.

 

 

 

Who you are:

The successful candidate must be able to establish and maintain collaborative and effective working relationships with coworkers, community partners and stakeholders. The CHW will have superior written and verbal communication skills and must be able to manage multiple tasks simultaneously, work well in team situations, meet deadlines, and work independently.

 

 

 

Our requirements for this role:

  • Experienced South Carolina Certified Community Health Worker. Certification as a South Carolina Community Health Worker II and/or South Carolina Community Health Worker Supervisor is required. Minimum 4 years of experience with at least 2 years in a supervisory or leading role.
  • Experience working with health care providers, coalition building, community organizing, population health, care coordination, or health care quality improvement.
  • Experience in developing and implementing a community care hub model or other integrated care models.
  • Strong understanding of community health issues, health care delivery systems, social determinants of health, and integrated care models.
  • A self-starter able to work independently with strong facilitation and motivational interviewing skills.
  • Experience delivering presentations and workshops.
  • High level of computer skills required, including high proficiency in Microsoft Suite (Word, Excel, PowerPoint, Outlook etc.) This position will require significant computer use.
